STRENGTHENING INTEGRATED CARE – A multi disciplinary approach to improving care for people with long-term conditions.

UNI are supporting SunWest Practice Based Commissioning Consortia to implement the Castlefields Integrated Care Model to improve care and quality of life for patients aged over 75 who have multiple long term conditions. The model enables a more individual level of support for this vulnerable group of patients, with the intention of preventing hospital admissions and reducing bed days through hospital in reach. The approach is based on a multi disciplinary team (including staff from both health and social care) working closely to improve the management of this group of patients across a practice population.

Practice teams are supported to use various methods to identify the patient group, including the EARLI tool, and to develop a register to ensure appropriate and timely follow up. The multi disciplinary nature of the team means that patients can receive the right intervention at the right time.

Over the next few months the participating teams will come together as a large group at least 3 times to see examples of best practice from other areas and to share ideas across teams collecting data along the way to demonstrate improved standards of care and more importantly improve independence and quality of life for these patients.

SPEAK OUT FOR SKIN – Community mobilisation in action.

UNI are working with communities and tissue viability teams in Grimsby to help mobilise local people to make a difference around skin breakdown – particularly around leg ulcers, diabetic foot ulcers and pressure sores. This exciting programme is part of a community response to tackle this silent epidemic that affects 200,000 people at any one time in the UK and which has a knock on effect amongst families and carers.  Donning placards and sandwich boards, local people outreach within supermarkets and local streets to raise awareness of symptoms and support people into local services. A wonderful array of social advertising approaches has been developed to support their work. Plans to link up with industry and hold Skin Oscars are also underway.

SHINE ON – Health Foundation award allows innovation in Manchester Nursing Homes.

UNI is supporting an innovative quality improvement programme across nursing homes in Manchester. The programme aims to create a replicable approach to facilitate multidisciplinary teams to provide nursing home residents with the right care at the right time.  Two waves of work will engage local staff, residents, their families and wider stakeholders. The programme is led by Manchester Community Health, with support from UNI and has received funding from the Health Foundation through the SHINE Awards.  The SHINE awards enable successful organisations to test innovative interventions that deliver high quality care.

Social Marketing in NHS Sefton

UNI are working with NHS Sefton to support a range of screening activity. The work will involve engaging with local communities to gather insight to develop social advertising materials and recommended approaches.  The work will cover cervical screening amongst young women, bowel cancer screening and diabetic retinopathy screening.
